Happy Pups, LLC Grooming Contract

·      I understand that Happy Pups has the right to refuse any service at their discretion.

·       I understand that there is always a chance of injury for my pet & that Happy Pups is not held liable for any injury while the pet is at the facility.

·       I understand that Happy Pups will put my pet’s health above the completion of the service and if Happy Pups feels unable to provide the service safely then the appointment may be cancelled.

·       I understand that all pets are required by law to be up to date on their Rabies vaccine. Dogs are highly recommended to have the Distemper and Bordetella vaccine, and to be on heartworm and flea and tick prevention. Cats are highly recommended to have the Feline Distemper vaccine and be on flea and tick prevention. If my pet is infested with fleas or ticks I understand that the appointment will be cancelled or charged a fee of $20.

·       I understand that there is a no show/ late cancellation fee of 50% of the scheduled appointment if not given a 24-hour notice. I understand that if I am 30 minutes late for my scheduled appointment time then the appointment is cancelled and considered a no show. I understand that there is a 3 no show policy & if you no show 3 times without a 24 hour notice then Happy Pups will no longer be able to service you or your pets in the future.

·       I understand that there will be $20 added per 30 minutes extra it takes to complete the service due to matting, coat length, difficult handling, etc.

·       I understand that the bill must be paid in full at time of pick up. If payment is not available no further services will be provided until the bill is paid and the account balance is $0.00.

·       I understand that any items brought may get lost or damaged and that Happy Pups is not responsible for any lost or damaged items.

Happy Pups agrees to exercise due and reasonable care to keep the facility sanitary and the condos enclosed. Happy Pups and its employees do not assume and shall not be held responsible for any liability with respect to the pet(s) listed in this agreement. Including loss by fire, theft, running away, death, injury to persons, animals or property, or death or injury to any other animal caused by the within named pet during the term and time of the contract. In no case shall Happy Pups by in any way liable or responsible. The responsibility and/or liability of Happy Pups, in no event shall exceed One Hundred dollars ($100.00) and the undersigned agrees to limit the responsibility against One Hundred dollars ($100.00) and no more, and agrees not to claim any damages against Happy Pups of any nature whatsoever, either by way of contract, equity, negligence or otherwise, in excess of said sum. Owner of said pet(s) specifically represents that he/she is the sole owner of said pet(s) and that the pet(s) has not been exposed to distemper, parvo, rabies, or other communicable diseases, within the last thirty days. If pet becomes injured Happy Pups will immediately notify the owner. Owner should pick up the pet(s) as soon as possible, if not able the owner agrees for Happy Pups to get the pet(s) veterinarian care and the owner agrees to pay any expenses for the care.
